Must Have:

• A completed application with 2 professional references
• Malpractice insurance 
• PA State Police Criminal History Record (PATCH): Apply online via the PATCH website 
• PA Child Abuse History Clearance: Apply online via the PA Child Welfare Portal
• FBI Fingerprinting Clearance
